Inleiding:
It is known that obese patients have more often chronic disorders such as diabetes and hypertension. However, it is still unknown if this also applies for common diseases presented in general practice.
Methode:
Within the Continuous Morbidity Registration (CMR), a Dutch general practice research and registration network database, a "nested study" was conducted. Patients diagnosed as ³obese² (Body Mass Index (BMI) 30 kg/m2 or more) and aged 20 to 75 years at the beginning of the study period, were compared with an at random selected population, without known ³overweight or obesity². The patients were matched for age, sex and socio-economic status. We studied the risk of presenting a common disease during a follow-up of five years.
Resultaten:
After a selection of the most incident morbidities, obese patients presented: common cold with fever (Incidence Rate Ratio (IRR) 1,59 [95% Confidence Interval (CI): 1,19-2,13]), common cold without fever (IRR 1,52 [95% CI: 1,25-1,84]), myalgia of the shoulder and neck region (IRR 1,57 [95% CI: 1,27-1,95]), dermatitis (IRR 1,34 [95% CI: 1,04-1,73]), mycoses (IRR 1,82 [95% CI: 1,42-2,32]), contusion (IRR 1,50 [95% CI: 1,12-1,99]) and lower urinary tract infections (IRR=1,60 [95% CI: 1,17-2,21]).
Besluit:
Besides the known chronic obesity related diseases, obese patients also present more common diseases to the general practitioner compared with normal weighted patients. With the growing prevalence of obesity, this will have far reaching consequences for the work load of the general practitioner.
