| published in Huisarts Nu 1998; 27 (9-10): 289-94 It is important that the GP explains a folate-rich diet to all pregnant women. Folic acid is found in raw vegetables, fruit, legumes and wholegrain products. However, this is insufficient. A supplement of 0.4 mg folic acid daily fromconception to the third month of pregnancy is necessary. Intake of 4 mg folic acid for the same period is recommended for women in the risk groups. Systematic detection of neural tube defects in all pregnant women by alphafetoprotein determination is not justifiable because of the high number of false-positive test results and the consequences this brings with it. The GP can give the pregnant woman the necessary and understandable explanation on the value and consequences of an alphafetoprotein determination. In the case of the high-risk groups, it is important to discuss the options for the test with the pregnant woman. The possibility of supplementing the diet with folic acid can be considered by the government.
